Follow the steps below to mail your application and see if you qualify for our Energy Support program. To save time, you can also apply online in minutes.

Step 1: Print Your Application

Download and print the Energy Support program application form. Double check that your printer does not cut off any information at the top or bottom of the page. You can also call our Care Center at (602) 371-7171 or (800) 253-9405 to have an application sent to you directly.

Step 2: Complete & Sign Your Application

Please complete and provide the information requested below. All customers listed on the APS bill need to sign the application.

Step 3: Gather Your Income Documents

View a list of all accepted income verification sources below in the "Program Eligibility Requirements" section. You should only submit copies of these income documents, as they will not be returned. All documents submitted are confidential and will be destroyed after processing is complete.


Step 4: Mail Your Application & Income Documents

Once you’ve completed the previous steps, please mail your application and all required documents to:

Please Note the Following

 

  • Applications without proof of income and/or signature(s) will be considered incomplete, and processing will be delayed or cancelled. We will notify you if your application is incomplete.
  • Please allow approximately 30 to 45 days for application processing if you apply by mail. If you would like your application to be processed sooner, apply online. We will notify you when your application has been approved.
  • To maintain eligibility, you must reapply for the program every 24 months. We will mail your re-certification application to you before your annual program start date.

Guidelines effective July 1, 2023. *Monthly gross household income is defined by the total household income, before taxes. This includes all money and non-cash benefits available for living expenses from all sources, both taxable and nontaxable, before deductions, including expenses, for all people who live in your home.

Proof of income guidelines

Below are examples of the accepted sources for income verification to enroll in our Energy Support program. 

Please provide copies of all sources of income for your household. Submit one of the following for the 60% or 25% discount.  

  • Last month’s pay stubs 
  • Last year’s W-2s  
  • Last year’s tax return (Federal Tax Form 1040, 1040A or 1040EZ) 
  • Social Security Disability Income 
  • Official child support or alimony award letters 
  • Veterans benefits 
  • Annuities 
  • Unemployment 

OR

Provide copies of one of your public assistance documents for the 25% discount.  

  • A copy of your EBT card 
  • A copy of your award letter from the Supplemental Nutritional Assistance Program (SNAP) 
  • A copy of your award letter from the Temporary Assistance for Needy Families program (TANF) 

Please only submit copies of your income documents as they will not be returned. All documents submitted are confidential and will be securely handled and destroyed after processing is complete. 

If you are applying online, you will want to take a photo of your documents and upload them during the application process.
