1. Panel members must be a current APS customer and at least 18 years of age.

2. Each customer is only allowed to create and engage in the activities as 1 panel member, with the feedback being your own.

3. The purpose of the panel is to learn about customers perceptions on key issues and get feedback about programs and initiatives APS is considering. Survey responses will not be anonymous, but results will be aggregated for analysis. Any survey responses associated with an individual will not be shared outside of APS.

4. APS will ask for demographic data, which it will use in aggregate to identify how to better serve different segments of customers. Your individual data will not be shared outside of APS. In some instances, your individual data may be used to respond to your inquiries or concerns or to connect you to products that you are likely to have interest in. All data will be protected in accordance with APS’s Privacy Policy.

5. Participation in the panel and any survey responses will not affect your APS service or bill.

6. Customers will be awarded "points" for completing surveys. The amount of points for each survey will be identified in the survey invitation. Once accumulating a certain amount of points, customers will redeem those points for an e-gift card to a major retailer of their choice. Individual terms for redemption of those gift cards will be stated upon selection. For any questions about the fulfillment of these rewards, customers can contact rewards@bhn.com

7. Customers can remove themselves from the panel by giving notice to CustomerPanelSupport@apsc.com or by clicking the "opt out" link in survey invitation emails.

8. APS may remove panel members at its own discretion.
