what information will I need to apply?


In order to successfully process your application, we will need the following:


  • ​APS account number and/or address of the location where the equipment will be installed
  • List of existing equipment and specification sheets for the equipment that is replacing it. Make sure the new equipment meets our specifications​
  • An invoice showing the quantity and model numbers of the new equipment and the total
